Question Number 1:
I am submitting a Freedom of Information request under the Freedom of Information Act 2000. Please could you provide the following information:

For questions 1, 2, and 5, could you please provide the data in table format, sorted by hospital and ward, with the total number of incidents per year up to and including this year?

- Total Falls by Hospital: The total number of inpatient falls by hospital, covering the last five years?
 
Answer To Question 1:
please see attached table. Be aware that we count all slips trips falls and assist to the floor as falls in our figures.

- Total Falls by Ward: The total number of inpatient falls by ward, covering the last five years?

Please see attached table. Be aware that we count all slips trips falls and assist to the floor as falls in our figures.

- Contact for Falls Leads: A list of NHS email addresses for falls leads/groups within the trust or the individuals responsible for fall prevention?

Unable to give this name due to banding.
 
Question Number 4:
- Falls Compensation Claims: The average annual compensation paid out by the trust in relation to inpatient falls over the past five years?

The Trust do not pay out compensation this goes via the NHSR - https://resolution.nhs.uk/
 
Question Number 5:
For questions 1, 2, and 5, could you please provide the data in table format, sorted by hospital and ward, with the total number of incidents per year up to and including this year?

- Absconded Patients by Ward: The number of inpatients who absconded from wards (defined as leaving without informing staff, resulting in a missing person status) by ward, covering the last five years?

Question Number 6:
- Absconding Compensation Claims: The average annual compensation paid out by the trust in relation to absconding incidents over the past five years?
 
Answer To Question 6:
The Trust do not pay out compensation this goes via the NHSR - https://resolution.nhs.uk/ unless they were under a DOLS and then died /were injured such a claim would be rare
 
Question Number 7:
- One-to-One Support for Fall Risk Patients: The average cost per patient, per day, for one-to-one support provided to patients at risk of falls?
 
Answer To Question 7:
The Trust will assess each patient for a risk of a fall on an individual basis and the amount of supervision will depend on this assessment. The Trust uses a combination of techniques to manage the patient in a safe manner. If 1:1 support were required then a Band 3 Healthcare Support assistant is likely to be used. The full cost of using a member of staff during normal weekly working hours, including on-cost, is between £15.14-£16.19 per hour The Trust does not maintain records to identify what the average cost per patient would be for a risk of fall on a daily basis.
